Abstract
This report describes our experience with the use of stent-grafts for the treatment of isolated iliac artery aneurysms (IAAs) and reviews the technical aspects of the procedure. Various stents were used in combination with polyester or polytetrafluroethylene graft material to treat 41 patients with peripheral vascular disease. Twenty of these procedures were for IAAs in 19 patients. There were 18 men and 1 woman, with a mean age of 72 years (range, 53 to 84 years). Fifteen of the 20 aneurysms were located in either the common iliac or the common and internal iliac arteries. Transluminal placement of the stent-grafts was successful in all patients. Immediate leaks were noted in four cases. Three were treated with additional stent-grafts and one with coil embolization with thrombosis of the aneurysms in all cases. There were no distal thromboembolic events or infections. The postprocedure course of two patients was complicated by thrombosis of the contralateral common iliac vein. The mean follow-up period was 21 months (range, 1 to 48 months). IAAs can be effectively treated with endoluminal placement of stent-grafts; however, long-term follow-up is needed before this mode of therapy can be recommended in all patients.
