A longitudinal study of parathyroid hormone and parathyroid hormone related peptide in pregnancy

OBJECTIVE: Measurement of maternal parathyroid hormone (PTH) bv iimtumoreactive assay has led to acceptance by standard texts that the level increases \\ith advancing gestation. Development of highly sensitive assays, however, now allows measurement of intact PTH and parathyroid hormone related peptide (PTHrP). We sought to determine PTH and PTHrP levels serialh through gestation and their relationship with calcium and magnesium. STUDY DESIGN: Serum PTH, PTHrP, ionized calcium (iCa) and magnesium (Mg) were measured during each trimester in 20 healthy gravidas. Levels were compared with those of In non-pregnant controls. A liighK sensitive iminuno-chemi-lumino-nietric assav was employed for determination of PTH and PTHrP. Analysis was by t-test and repeated measures ANOVA with p < 0,05 considered significant. RESULTS: PTH but not PTHrP levels were significantly lower in pregnant than non-pregnant subjects (1.35 pmol /L ±.19 vs 2.57 pmol/I. ± .20, p < 0.001). PTH did not van significantly between trimesters but trended toward a second trimester decrease (1.35 pmol/I. ±0.19, 1.0 ± 0.12, 1.29 ±0.17. p = 0.06). There was no difference in PTHrP across trimesters. Ionized ("a did not change but Mg decreased with advancing gestation (p < 0.005). PTH and PTHrP did not correlate with iCa in any trimester. CONCLUSIONS: These data suggest that intact PTH and PTHrP, and iCa levels do not change with advancing gestation.
